The honor roll is made of marble and is set inside a distinctive timber frame with a classical pediment at the top. The names are inscribed on the marble and highlighted in gold. 

The roll was unveiled at the Frederickton School of Arts on the evening of 24 May 1922 by Mrs Charles Doran, a mother of three soldiers, two of whom fell in the war. The occasion was reported by The Macleay Argus on 30 May and The Macleay Chronicle on 31 May. Both articles described the function in detail, as well as listing those named on the roll. The Argus stated an audience of some 300 people were in attendence and mentioned two captured machine guns were also unveiled on the night.

Discussion of the roll began several years earlier at the annual meeting of the Frederickton School of Arts, on Wednesday 11 July 1917 (The Macleay Chronicle, 11 July 1917). The establishment of the roll had been started by the Frederickton Football Club and the School of Arts Committee also became involved. 

Access to the memorial is restricted to the opening hours of the School of Arts building.
