This memorial is a bushrock bearing a bronze honour roll for the First and Second World Wars, the Malaysian Emergency, and the Vietnam War. Attached to the rock above the plaque is the joint services emblem, which represents the Australian Army, Royal Australian Navy, and the Royal Australian Air Force. A flagpole has been installed near the rock so flags can be flown during commemorative services, which take place on site.
On 26 March 2017, then Deputy Prime Minister of Australia Barnaby Joyce reported Woolbrook Hall and Park Committee received an Australian Government grant to honour the service and sacrifice of local servicemen and women. The committee received two grants, of $4,000 and $1,500, to refurbish and update two honour rolls displayed within the nearby Woolbrook A.I.F. School of Arts Hall and install this new outdoor honour roll with flagpole. The grants were part of the Australian Government's 'Saluting Their Service' program, to help communities commemorate the contributions of local service people.
The memorial was dedicated in 2018. Names on the plaque also appear on the Woolbrook First World War Roll of Honour.
