Attached to its front face is a plaque dedicated to Squadron Leader John Holt and Flight Lieutenant Adam Noordink, who were killed after their aircraft was involved in a bird-strike incident on 29 September 1977. They were flying from the Amberley RAAF base to the Evans Head Air Weapons range. The memorial is located in the Evans Head War Cemetery. Various commemorative services have been held at the cemetery since its opening. Inscription Plaque In memory ofSquadron Leader John Francis HoltFlight Lieutenant Adam Philip Winold Noordinkof No. 6 Squadron who tragically lost their lives in the crash of a Royal Australian Air Force F-111C, A8-133 on 29 September 1977. 'Falcon 32' crashed whilst on a practice bombing mission at Evans Head Range. A8-133 was struck by large birds on the canopy cockpit causing the aircraft to crash south of the range with the loss of both aircrew. They made the ultimate sacrifice serving their country.
