The page features an illustration at the top, containing various symbols representing military service and the British Empire. These include Britannia holding a laurel wreath, a soldier and sailor, the Union Jack and Australian flags, and the First Commonwealth Coat of Arms. A ruled table, with columns for name and rank, fills the main body of the page.
