The book has a dark blue leather cover with the inscription in gilt. Each page contains a photograph of a Second World War veteran and handwritten information on their service. The information came from the veterans' families or the veterans themselves; however, the process for collecting names is not clear. The memorial does not include all veterans who attended the Parkes Methodist Church, now the Uniting Church. 

The book is located inside the Parkes Uniting Church. Also on site is the Parkes Methodist Church First World War Roll of Honor.

The Book of Remembrance was dedicated at a special service, conducted by Reverend R B Lew, President of the Conference, in the Parkes Methodist Church on Sunday 27 November 1949. The book commemorated three members of the congregation who gave their lives, and those who served in the Second World War. Several items of church furnishings were also dedicated, including a stained glass memorial window, additions to the reredos panelling along the back wall, a lectern, a sanctuary chair, a hymn board and cupboards, the sanctuary and aisle carpets and a porch table (The Methodist, 16 December 1949).
