The honour roll is St Andrews Anglican Church in Coolah is a decorative white marble tablet, attached to a black marble backing. It lists the names of those from the church who served in the First World War. Each man who lost his life is marked by a 'K'. The top of the tablet features the crossed Union Jack and Australian flags and foliage. 

The Mudgee Guardian and North-Western Representative, 17 October 1918 reported that a Market and Sports Day had been organised to raise funds for the erection of the church's roll and to complete repairs to the building. The report noted that the whole of the exercise was due to the members of the Ladies Guild, with support from several individuals and the Parochial Council.

It was not until the middle of 1919 that the Guardian (15 August 1919) reported that the roll had been installed the previous week. The work of making, engraving and erecting the roll was skilfully completed by Mr D.H. Acton of Mudgee and Lithgow. The newspaper also noted that arrangements for the unveiling ceremony would shortly be announced by the Rector Rev. W. Hotson. It is believed that the service may have been undertaken on 14 September.
