The memorial is a Gothic-shaped, timber roll of honour, featuring solidly carved columns on each side. At the top of the board are colour paintings of the crossed Australian Red Ensign and Union Jack flags. In between the flags are a five-pointed star and a blue shield featuring the cross of St George. Beneath the shield is a banner with the words 'Advance Australia'.

The roll is for members of the Alstonville Methodist Church who served in the First World War. Their names are printed in gold in a table on the lower half of the board. The table has two columns, which are headed 'Names' and 'Remarks'. Four veterans have remarks against their names, three for being Killed in action and one for having Retired/Returned due to illness. 

The origins of this memorial are unknown; however, it may have been established around 1918. An Anzac service was held at the church on 28 April 1918 during which "the roll of honour was read, and silent and audible prayers were offered" (Northern Star, 1 May 1918). This may indicate the memorial was in the church at the time. 

The church is now known as the Alstonville Uniting Church and access to the memorial is restricted to its opening hours. 

In 1977, the Uniting Church was created with the merging of the Presbyterian, Methodist, and Congregational Churches.
