The memorial is a large oak honour roll with the names of those who served written in gold across eight columns.

On 7 August 1920, The Uralla Times and District Advocate, reported it was intended to install an oak altar and honour roll in St John's Church, Uralla as a tribute to the members of the Church who fought in the Great War. At that stage, some 130 names had been collected. The newspaper editions of 2 and 9 February 1922 listed the names placed on the honour roll and described the dedication ceremony which took place on 5 February 1920 in detail. The Armidale Bishop dedicated the honour roll and Lieutenant R.H. Blomfield unveiled it.
