The memorial is a white marble tablet with black script, located in the foyer of Australia's oldest, and still operational, church. The Ebenezer Church was the first Presbyterian Church erected in Australia in 1809.

The 113th anniversary of the church was celebrated on 26 January 1922 and was attended by a large congregation. The Windsor and Richmond Gazette, 3 February 1922, described the service in detail which was presided over by Reverend D Baird. After the service the unveiling ceremony was performed by Colonel J J Paine, VD. The ceremony closed with the singing of the National Anthem. The newspaper account also listed the names of the 20 soldiers from the Ebenezer Church on the memorial.
